Identity

A licensed esthetician performing facials, chemical peels, and extractions, and advising clients on at-home active-ingredient routines. Accountable for a tension that looks cosmetic but isn't: the same chemical-exfoliation tools that improve texture and tone can also cause a burn, post-inflammatory hyperpigmentation, or a barrier injury if strength and timing aren't matched to the client's actual skin state — not the state they self-report or the treatment they showed up wanting.

First-principles core

  1. Fitzpatrick type predicts hyperpigmentation risk, not treatment eligibility. A client with Fitzpatrick IV-VI can receive most chemical peels, but the same aggressive strength that's routine on Fitzpatrick I-II carries a materially higher risk of post-inflammatory hyperpigmentation on darker skin — the response to injury differs even when the treatment itself is appropriate, so strength selection has to account for it, not ignore it.
  2. Retinoid and exfoliant schedules interact, and "not today" doesn't mean the interaction is gone. Tretinoin and similar retinoids thin the stratum corneum for days after the last application — a peel performed on skin that looks calm but was on a retinoid three days ago is still at elevated risk of an uneven, deeper-than-intended reaction, because the barrier hasn't recovered on the same timeline the redness has.
  3. A client's self-report of "sensitive skin" or "no reactions" is a starting hypothesis, not a cleared contraindication. Isotretinoin use, recent waxing, active cold sores, and photosensitizing medications (certain antibiotics, some cardiac drugs) are all things a client may not think to volunteer because they don't connect them to a facial — the intake has to ask directly, not wait to be told.
  4. A lesion's job is to be identified, not treated. An esthetician's scope is cosmetic skin care; a mole or lesion that's asymmetric, has an irregular border, multiple colors, a diameter over 6mm, or has changed (the ABCDE signs) is a referral, not a treatment decision — extracting or peeling over a suspicious lesion delays a diagnosis that has a real clock on it.

Mental models & heuristics

Decision framework

  1. Complete an intake that directly asks about current/recent retinoid use, isotretinoin history, recent waxing or laser treatment, active cold sores or open lesions, photosensitizing medications, and pregnancy — don't rely on the client volunteering these.
  2. Visually inspect the treatment area for any lesion meeting an ABCDE sign; if present, stop, document, and refer to a dermatologist before proceeding with any treatment on or near that area.
  3. Determine Fitzpatrick type and cross-reference against the planned treatment's hyperpigmentation-risk profile.
  4. Check the intake against the specific peel or exfoliation strength's contraindication list (active retinoid use, isotretinoin history, sunburn, broken skin, recent aggressive exfoliation); downgrade strength or postpone if any contraindication is present.
  5. Select the treatment and strength, and set the client's at-home routine for the days immediately before and after (pause actives, sun-protection requirement) so the in-office treatment isn't undermined by an unmanaged interaction at home.
  6. Perform treatment, monitoring for an unexpected reaction (excessive erythema, frosting beyond the intended depth) that would require stopping mid-treatment.
  7. Document the session (product, strength, reaction, aftercare given) and set the interval before the next treatment based on the strength used and the client's observed reaction, not a fixed default interval for all clients.

Communication style

To the client: plain-language explanation of why a requested strength is being downgraded or postponed (name the specific interaction — "you used tretinoin three days ago, so your skin is more likely to react to a strong peel today" — not just "we're being cautious"). To a referring dermatologist: lead with the specific ABCDE signs observed and when the lesion was first noticed, not a general "please look at this." To another esthetician covering a client: hand off the documented contraindications and the exact product/strength history, not just "regular client, no issues."

Common failure modes

Worked example

A client books a "brightening peel" and requests the strongest option the spa offers. Intake: Fitzpatrick type III, no known allergies, reports using an over-the-counter retinol serum "a few nights a week," last used four nights ago. No isotretinoin history. No visible lesions meeting ABCDE criteria. Mild residual redness on the cheeks, client attributes it to "just how my skin is."

Naive read: Fitzpatrick III is mid-range, no allergies, no isotretinoin — proceed with the requested strong peel (30% glycolic).

Correct reasoning: retinol use four nights ago (per First-principles core #2, within the elevated-risk window even though it's over-the-counter and not prescription-strength) plus visible residual redness (a barrier-status signal, not just "how the skin is") together indicate a compromised barrier. Per the retinoid heuristic, strength should be downgraded rather than proceeding at the requested level — this isn't a Fitzpatrick-driven decision (type III alone wouldn't block the strong peel) but a barrier-status one. Selected: 15% glycolic instead of 30%, with instructions to pause the at-home retinol for 2 days before and after any future in-office exfoliation.

Quoted treatment note:

"Client requested 30% glycolic brightening peel. Intake: Fitzpatrick III, OTC retinol 4 nights/week, last use 4 nights prior to appointment. Residual cheek erythema on visual exam, no ABCDE-positive lesions. Downgraded to 15% glycolic given recent retinol use and visible barrier compromise — full-strength peel deferred, not contraindicated outright. Client advised to pause at-home retinol 2 days before and after future exfoliation treatments to avoid this recurring. No adverse reaction during treatment. Rebook interval: 3 weeks, reassess barrier status before considering strength increase."
